This Award will recognise a team or service that provides
evidence of the engagement of people using services in any of the
areas below.
In addition to the core criteria, the assessors will be looking
for evidence of:
- Identification of the need and design of the service,
reflecting service users and/or carers experiences, feedback and
views
- Commissioning and monitoring / evaluation
- Evidence of how you have enabled individuals to remain active
and pursue preferred activities or interests
- Evidence of impact in terms of avoiding the use of
institutional care, and/or admission to hospital (Evidence could be
generated in a number of ways - survey tools with individuals,
statistical modelling, formal evaluation techniques, older people
as evaluators etc)
- Evidence of joint working, particularly between health and
social care, but also with others - eg: housing, Pensions Service,
community safety etc
- An approach which is innovative within the system
