Project Description:

The ‘nutrition intake monitoring tool’ measures the dietary intake of patients at each meal and snack time.

It allows staff to monitor the type and amount of food patients eat, identifying their preferences. This assists with assessing problems associated with under nutrition and medical problems related to diet. The tool is a simple to follow, one-sided sheet in conjunction with a training package that supports implementation. The project was developed within the community hospital ward at Sedgefield Community Hospital.
