The PCTT grades adverse events using an internationally recognised system. It begins at level D to exclude very minor events.
D reached patient and required monitoring to confirm it resulted in no harm +/- required intervention
E contributed to or resulted in temporary harm to the patient and required intervention
F contributed to or resulted in temporary harm to the patient and required initial or prolonged hospitalisation
G contributed to or resulted in permanent patient harm
H required intervention to sustain life
I contributed to the patient’s death
