Joined-up care

As patients pass through boundaries within and between organisations on their healthcare journey, there is often duplication, inefficiency and waste that can create a poor experience. At these boundaries between and within organisations - working practices can become disjointed, and this can also lead to frustration for staff and patients. 

Focussing on how different services can work together for patients, has a huge potential to increase the quality of care provided, reduce waste and consequently the cost of providing care.  Joining services together to make a smooth system that is easy to navigate is our challenge - and Joined-up care aims to meet this.
